If you opening images into the camera raw dialog before going to photoshop, check the Resolution settings in the Workflow Options(bottom middle of the camera raw dialog)

Sounds like you're using Dynamic Link? Don't. From Premiere, export using Media Encoder. Use the "MPEG-2 DVD" format and an appropriate preset, such as "NTSC widescreen" or whatever suits your needs....
