Advanced Actions Slide

I have created a slide and some advanced actions.  Could you have a look and tell me what is missing and why it has done what it has done?

Here is the slide.  The background is a .png from a math problem:

Here are my two Advanced Actions:

When I click on preview, "From this slide", I get:

Can you tell me what happened to to my problem background and why only the check mark, TEB, and Submit are left on the screen?  Also, when I do Advanced Actions with a TEB, do I ignore the box that opens and asks for the value and just use Advanced Actions to specify what the answer should be?

I know I am doing something wrong, but knowing what it is should help me understand the process.  Thanks!
